A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF WEST
COVINA, CALIFORNIA, THAT THE BUILDING OFFICIAL SHANNON
YAUCHZEE, CITY ENGINEER THOMAS MAYER, OR PUBLIC
SERVICES DIRECTOR JEFFREY COLLIER ARE HEREBY
AUTHORIZED TO EXECUTE FOR AND IN BEHALF OF THE CITY OF
WEST COVINA, A PUBLIC ENTITY ESTABLISHED UNDER THE LAWS
OF THE STATE OF CALIFORNIA, THIS APPLICATION AND TO FILE IT
IN THE OFFICE OF EMERGENCY SERVICES FOR THE PURPOSE OF
OBTAINING CERTAIN FEDERAL FINANCIAL ASSISTANCE UNDER
P.L. 93-288 AS AMENDED BY THE ROBERT T. STAFFORD DISASTER
RELIEF AND EMERGENCY ASSISTANCE ACT OF 1988, AND/OR
STATE FINANCIAL ASSISTANCE UNDER THE NATURAL DISASTER
ASSISTANCE ACT.
WHEREAS, the City of West Covina has made an application and has been
obligated funding for Hazard Mitigation administered by the Governor's Office of
Emergency Services (OES); and
WHEREAS, in an effort to minimize the paperwork associated with receiving
disaster assistance, OES has implemented revisions to existing Public Assistance
documentation requirements; and
WHEREAS, the OES 130 form has also been modified to identify authorized
agents by title and as a result of this change, applicants will no longer have to submit a
new OES 130 when personnel changes occur within the named positions; and
THAT the City of West Covina, a public entity established under the laws of the
State of California, hereby authorizes its agent(s) to provide to the State Office of
Emergency Services for all matters pertaining to such state disaster assistance the
assurances and agreements required.
