Staff is proposing to maintain the current assessment rates for the Fiscal Year beginning July 1, 2002. The City Council should now hold the required hearing to levy and collect assessments. The total amount to be assessed is $772,926. This ordinance is now being presented for second reading and adoption. Lopez and Tamara R. Silva -Lopez vs. City of West Covina On August 2, 2001, the City Clerk's Office received a claim for personal injuries in the amount of $500,000 relating to an incident with the West Covina Police Department. b) Cathy Rodriguez vs. City of West Covina On March 7, 2002, the City Clerk's Office received a claim for property damage in the amount of $245. Claimant is seeking to recover costs she incurred to replace a broken window on her neighbor's vehicle. c) Roy Molina vs. City of West Covina On December 27, 2001, the City Clerk's Office received a claim for personal injuries relating to an incident at the West Covina City pool. The amount of claim had not been determined at time of presentation. d) Continental Casualty Company vs. City of West Covina On January 16, 2002, the City Clerk's Office received a subrogation claim for monetary loss relating to an incident at the West Covina City pool. The amount of claim had not been determined at time of presentation. e) .James A. Caruso vs. City of West Covina On February 8, 2002, the City Clerk's Office received a claim for personal injuries in the amount of $4,000 relating to a trip and fall accident on a public sidewalk. Claimant is seeking to recover money for his alleged pain and suffering. f) Farmers Insurance/Caryll Annette Honor vs. City of West Covina On March 29, 2002, the City Clerk's Office received a claim for property damage in the amount of $4,343. Garvey Avenue South Accept onsite and offsite improvement for Precise Plan No. 872 and authorize release of Bond No. 815641S by Developers Insurance Company in the amount of $154,000.00. b) Principal: Rielly Homes Location: Tract No. 32324 and Tract No. 47809 (South Hills) Accept onsite and offsite improvements for portions of Tract No. 32324 and Tract No. 47809 and authorize the reduction of Bond No. B2973121 by United Pacific Insurance Company from $350,000.00 to $160,000.00. The balance of the bond will remain in effect until the end of the one-year plant establishment period of the landscape area with Maintenance District No. 7. 6) Traffic Committee Minutes of May 21, 2002 (accept & file) I. A request by the West Covina Unified School District that adult crossing guards be considered at the intersections of Valinda/Vine, Valinda/Merlinda, Conlon/Durness, Evanwood/Vine, Alwood/Tamar, Banbridge/Belmont, Willow Avenue/Farlington Avenue, and the Mid - block school crosswalk on Wednesday Drive, east of Valerie court resulted in the following Traffic Committee recommendation: That assigning of adult crossing guard at the intersection of Valinda Avenue and Vine Avenue is not justified based on the State Traffic Manual guidelines. That assigning of adult crossing guard at the intersection of Valinda Avenue and Merlinda Avenue is not justified based on the State Traffic Manual guidelines. That the survey for the consideration to place an adult crossing guard at the intersection of Conlon Avenue and Durness Avenue be performed upon the commencement of school in September 2002. That assigning of adult crossing guard at the intersection of Vine Avenue and Evanwood Avenue is not justified based on the State Traffic Manual guidelines. That the assigning of adult crossing guard at the intersection of Alwood and Tamar Avenue is not justified based on the State Traffic Manual guidelines. A City initiated request that parking along Glendora Avenue between Lakes Drive and Dalewood Avenue be prohibited on Thursdays from 3:30 p.m. to 10:00 p.m. from May through November of each year for the purposed of conducting the City's weekly Street Fair resulted in the Following Traffic Committee recommendation: That parking along Glendora Avenue between Lakes Drive and Dalewood Avenue be prohibited on Thursdays from 3:30 pm. to 10:00 p.m. from May through November of each year for the purpose of conducting the City's weekly Street Fair. [II. A request by Hertz Car Rental that parking be prohibited along South Garvey Avenue, west of Hollenbeck Avenue resulted in the following Traffic Committee recommendation: That red curb be installed along both sides of Garvey Avenue South, west of Hollenbeck Avenue. IV. A request from West Covina resident Luis Castillo that traffic conditions on Vine Avenue between Glendora Avenue and Lark Ellen be reviewed resulted in the following Traffic Committee recommendation: That this item be held over to provide staff time to gather the necessary data. 7) Award of Bids a) Historical House Structural Repairs Proiect No. BP-02312A — Award of Bid This project provides for structural repairs to be made to the Historical House at Heritage Park. At the request of the City Council, Hess Engineering, Inc. completed a structural survey of the facility and on March 5, 2002, the City Council directed staff to seek bids to have the work done. It is recommended that the City Council approve the repairs listed in Line Items Nos. B-1 and B-2 to the Historical House at Heritage Park as identified by the firm of Hess Engineering, Inc. and take the following actions: 1. Approve the expenditure of $9,350 from City Buildings Termite Repair (110-320-3001-7500) and $12,840 from Park Buildings Termite Repairs (161-320-3201-7500) for this project. 2. Approve an appropriation of $19,800 from City Yard Fuel Island (110- 320-3769-7900) to be transferred into Account No. 110-320-3294- 7500 to be used for this project. 3. Approve an appropriation of $28,100 from Construction Tax Fund balance to be transferred into Account No. 161-320-3294-7500 to be used for this project. 06.04.02 7 Council Agenda 06/04/02 • 7) Award of Bids (continued) a) Historical House Structural Repairs (continued) 4. Authorize staff to retain Jeff Coronado Structural Engineering consultants (JCSEC) of West Covina, California, to perform structural observation for this work, 5. Accept the bid of Pacific Construction in the amount of $63,300.00 as submitted at the bid opening on April 30, 2002, and authorize the Public Works Director/City Engineer and City Clerk to execute an agreement with Pacific Construction Company of Granada Hills, California, for the recommended work to be done. b) Purchase of One Truck — Award of Bid The Maintenance Division of the Public Works Department is requesting approval to replace one aged utility truck with one new utility truck. It is recommended that the City Council take the following actions: 1. Award the bid for one 2002 Chevrolet 2500 HD 4X2 Regular Cab and Chassis with a Utility Body per specifications in Bid No. 01-02-156 to West Covina Motors, Inc. of West Covina, including tax and delivery, not to exceed $23,050. 2. Declare old Unit No. 156 as surplus and send it to public auction per WCMC Sec. 2-316 after the new 2002 Chevrolet 2500 HD 4X2 Regular Cab and Chassis with a Utility Body is put into service. c) Purchase of Medical Re -Supplies The Fire Department is requesting the purchase of medical re -supplies. It is recommended that the City Council take the following actions: 1. Award Life -Assist the contract for the purchase of ALS/BLS medical supplies and equipment for the West Covina Fire Department; and 2. Authorize the Fire Chief to purchase medical supplies equipment from Life-Assit; and 3. Authorize the maximum expenditure of $36,000.00 from Account No. 110-320-2181-6232 and a maximum expenditure of $50,000.00 from Account No. 110-320-2181-6270 for these purchases. d) Proposal for Capital and Infrastructure Inventory and Valuation Services — Award of Bid This is a request that the City Council award a professional services contract to the firm of Conrad Business Services, Inc. for capital and infrastructure inventory and valuation services at a cost of $59.930. It is recommended that the City Council accept the bid of Conrad Business Services, Inc. in the amount of $59,930 and authorize the City Manager to execute an agreement with them for the service to be performed per the City of West Covina specification 0102A, subject to review and approve of the City Attorney. It is further recommended that the City elect to depreciate infrastructure assets, to include all infrastructure assets in the inventory, and to track the infrastructure assets in appropriate networks or subsystems. 06.04.02 8 Council Agenda 06/04/02 • • 8) Miscellaneous — Environmental Management a) Graffiti Removal Agreement Renewal The current Agreement with the Boys and Girls Club of San Gabriel Valley for graffiti removal services in the City of West Covina expires on August 31, 2002. Amendment No. 7 extends the contract period for another year. It is recommended that the City Council authorize the City Manager and City Clerk to execute Amendment No. 7 to the Graffiti Agreement, extending the existing contract to August 31, 2003. Miscellaneous — Police Department b) Donation of 1978 S.W.A.T. Truck to the City of Placerville The Police Department is requesting City Council approval to donate a surplus S.W.A.T. truck to the City of Placerville California . It is recommended that the City Council authorize the donation of the 1978 Ford S.W.A.T. truck (license E725112) to the City of Placerville. The transaction will be completed upon receipt of a one -dollar purchase order. The lowest responsive bid is $779,850 ($155,970 annually) from R.F. Dickson of Downey, California. It is recommended that the City Council accept the bid from R.F. Dickson Company in the amount of $779,850 ($155,970 per year) for a five-year period as submitted at the bid opening of May 14, 2002, and authorize the Public Works Director/City Engineer and City Clerk to execute an agreement with R.F. Dickson of Downey, California. It is recommended that the City Council authorize Community Services staff to organize a community meeting at Shadow Oak Park and obtain input from residents regarding their vision for the future of Shadow Oak, particularly in reference to the undeveloped lower level area.
